Youth:

    * Physical form signed by a doctor within 12 months 
      If you are not sure if your last physical is within the 12 month window, we will have a binder
      at upcoming troop meetings of all the physicals we brought with us to camp last year so you 
      may check whether or not you need to get a physical this year.
    * Medical Form Supplement (needs to be done by everyone)
    * A copy (front & back) of your family insurance card

Adult:

    * Adults over 40 years old MUST have a physical form signed by a doctor within 12 months (if under 40, refer to youth)
    * Medical Form Supplement (needs to be done by everyone)
    * A copy (front & back) of your family insurance card

 
ALL SCOUTS, REGARDLESS OF CURRENCY OF PHYSICAL, MUST COMPLETE A SCOUT INFORMATION FORM AND A NEW MEDICAL SUPPLEMENT FORM
 

Prescription Medications:

    * If you are bringing prescription medications to camp, fill out the Prescription Medication 
      Information Form for each drug brought. This form should be attached to the scout's physical exam.
    * Next, fill out a Prescription Medication Card for each meal or time that medications will be taken
      throughout the week. Place the card and medication(s) in a small ziploc bag. For example, Joe Scout
      only takes one prescription medication and he takes the medication each morning with breakfast. 
      Therefore, his guardian should fill out 6 cards and place the cards into 6 ziplock bags with 6 doses 
      of the medication.

As Needed Medications:

The Camp Health Lodge stocks a wide variety of over the counter medications; therefore, you are encouraged to
leave medications such as Tylenol or cough syrups at home. However, we realize that some participants
use medications that are used on an as needed basis such as Claritin. 
Please Fill out an "As Needed Medication Card" for As Needed Medications or Over the Counter Medications.
Fill out an "As Needed Medication Card" for each medication brought to camp and place all the cards and 
medications in one ziplock bag.
